Epidemiology of Chronic Critical Limb Ischemia

چکیده
Peripheral artery disease (PAD) is often referred to as a continuum of disease of occlusive arterial syndromes that can range from asymptomatic obstructive disease through occlusive disease requiring amputation. This entire spectrum of PAD has prevalence as high as 20 % of the general population [ 1 ]. This spectrum becomes more progressive and symptomatic as the disease causes an imbalance of distal perfusion pressure to the tissue and metabolic demands within that tissue. On the latter end of this continuum, chronic critical limb ischemia (CLI) has a prevalence that is more diffi cult to defi ne and is quite variable in the published literature. Unlike asymptomatic PAD or exertional claudication, CLI occurs with inadequate perfusion at rest [ 2 ]. Like most of the terminology of peripheral vascular disease, the defi nitions of CLI have evolved over the years, with fi rst an increasing need to classify the entire continuum of PAD, the need to further classify those undergoing surgical procedures, and then to include more objective measures as well as the clinical presentation. In this chapter, we will discuss the epidemiology of CLI. We will present the historical background of CLI and the risk factors along with its clinical presentations and then after the epidemiology and prevalence of CLI along with its risk stratifi cation and prognostic data before discussing the socioeconomic impact of this disease. Critical Upper Limb Ischemia Due to Brachial Tourniquet in Misdiagnosed Thoracic Outlet Syndrome after Carpal Tunnel Decompression: A Case Report
We present the case of a 68-year-old woman, referred to our department for critical upper limb ischemia, which had occurred a few days after homolateral surgical ligamentotomy for carpal tunnel syndrome, diagnosed and confirmed by electromyography, and performed with a brachial tourniquet. The patient was later admitted for subsequent progressive necrosis of the first three fingers of the left ...

Peripheral artery diseases amount to more than 20% of all types of cardiovascular diseases, which corresponds to 2-3% of the total population. 15 to 30% of the population of Russian Federation over 65 years have signs of obliterating arteriopathy. Advance of arterial insufficiency in some cases leads to development of special state - chronic critical limb ischemia (CLI). This article presents a...
متن کاملImproving the level of amputation of the lower extremity after therapeutic endovascular intervention for patients with critical limb ischemia
Methods: We conducted a comparative study of two groups of patients suffering from arterial insufficiency of the lower extremities, and the decision taken to amputate above the knee for both of them. A group of patients selected for a therapeutic peripheral intervention, and the effect of this intervention in improving the level of amputation studied and calculated statistically. The study cond...

Critical limb ischemia (CLI) is a growing epidemic with bleak patient outcomes. A variety of treatment modalities have been adopted to address CLI based on comorbidities, life expectancy, and the nature of the arterial disease. With advances in technology and treatment strategies, the clinical outcomes of CLI patients have significantly improved over recent years. However, despite progress, pat...
متن کاملThe Effect of Iloprost on Renal Function in Patients with Critical Limb Ischemia☆
BACKGROUND Iloprost, which has efficacy in the microvascular space, is shown to have beneficial effects on the kidney, which has an extensive microvascular network. OBJECTIVE We aimed to evaluate the effect of iloprost treatment on kidney functions in patients with critical limb ischemia. METHODS Forty-eight patients with critical limb ischemia who were not suitable for revascularization an...
